The Darien Clinic and Clinton Clinics are now scheduling physicals during July and August. Physicals are offered at a special rate of $25 on a series of dates and include a free Under Armour sports bag, while supplies last. (Cost is for physical and excludes lab fees, x-rays, and immunizations.)
Parents or guardians must accompany the child and bring immunization records. The Wisconsin “Green Form” is necessary and patient portion completed prior to appointment if your child is having a sports physical.
